Sorority And Fraternity Report Form
If an incident has occurred with an individual or student organization, you can report that here. This form will automatically be shared with the IFC, PAN, and MGC standards/policy chair. If needed, this report will also be shared with university standards such as WSU Center for Community Standards.
WSU Request For Student Care and Support
If you think a student attending WSU is in crisis or needs support, fill out this form. If the student is overwhelmed by stress, or in active distress or crisis, you can request the university to take action in supporting this individual.
WSU Formal Complaint Form with Compliance and Civil Rights
If you have experienced any violations that includes harassment or discrimination with a student or organization, WSU will investigate the matter.

The community standards process is designed to support students, uphold their rights and responsibilities, and hold them accountable for behaviors that conflict with our community standards. If you have experienced any violations that correlates to an individual or individuals then fill out this form.
